Abstract: This article, published in 2017 in Current Opinion in Pharmacology by Elsevier, gives a basic explanation about topical inhalation versus systemic delivery through inhalation.

Several examples are given for products targeted to the central airways, the peripheral airways, for delivery to the heart, the brain or the systemic circulation. Systemic inhalation can avoid a first pass effect by the liver, and it can result in a fast onset of drug action.

For such applications it is very important to make use of reliable, user-friendly inhaler devices which are designed to remove patient error.
